HIV: Έγκριση της ΕΕ στο πρώτο ολοκληρωμένο σχήμα δισκίου με βάση τη δαρουναβίρη

Η Janssen-Cilag International NV (Janssen) ανακοίνωσε πρόσφατα ότι η Ευρωπαϊκή Επιτροπή ενέκρινε τη χρήση του D/C/F/TAF1, ενός σχήματος ενός δισκίου (STR) με βάση τη δαρουναβίρη που χορηγείται μία φορά την ημέρα, για την αντιμετώπιση της λοίμωξης από τον ιό της ανθρώπινης ανοσοανεπάρκειας τύπου 1 (HIV-1) σε ενήλικες και εφήβους ηλικίας 12 ετών και άνω με σωματικό βάρος τουλάχιστον 40 kg. Ο γονοτυπικός έλεγχος θα πρέπει να κατευθύνει τη χρήση του D/C/F/TAF. Η κομπισιστάτη, εμτρισιταβίνη, και η τενοφοβίρη αλαφεναμίδη είναι προϊόντα της Gilead Sciences, Inc.

Το D/C/F/TAF, το μοναδικό STR με βάση τη δαρουναβίρη που ενδείκνυται για τη θεραπεία αυτής της ομάδας ασθενών, συνδυάζει την αποδεδειγμένη και μακροχρόνια αποτελεσματικότητα της δαρουναβίρης με το βελτιωμένο προφίλ των νεφρικών εργαστηριακών εξετάσεων και της οστικής πυκνότητας του συνδυασμού F/TAF σε σύγκριση με τον συνδυασμό F/TDF (εμτρισιταβίνη / φουμαρική τενοφοβίρη δισοπροξίλη). Είναι η μόνη εγκεκριμένη θεραπεία που προσφέρει τα πλεονεκτήματα ενός STR, σε συνδυασμό με τον υψηλό γενετικό φραγμό στην ανάπτυξη αντοχής που προσφέρει η δαρουναβίρη.

Η διαθεσιμότητα θεραπευτικού σχήματος ενός δισκίου, με υψηλό φραγμό στις μεταλλάξεις αντοχής, περιορίζει την ανάγκη χορήγησης ξεχωριστών δισκίων, μειώνοντας την επιβάρυνση από τη λήψη δισκίων στην καθημερινότητα των ασθενών, και βοηθώντας τους να επιτύχουν καλύτερη συμμόρφωση στη θεραπεία και ιολογική καταστολή», είπε ο Jean-Michel Molina, Καθηγητής Λοιμωδών Νοσημάτων στο Πανεπιστήμιο Paris Diderot.

Τα αποτελέσματα μίας μελέτης βιοϊσοδυναμίας που συνέκρινε το D/C/F/TAF με τη συνδυασμένη χορήγηση των ξεχωριστών παραγόντων δαρουναβίρης [D] 800 mg, κομπισιστάτης [C] 150 mg, και του συνδυασμού σταθερών δόσεων εμτρισιταβίνης /τενοφοβίρης αλαφεναμίδης [FTC/TAF] 200 mg/10 παρουσιάστηκαν τον Ιούλιο2 στο συνέδριο της Διεθνούς Εταιρείας για το AIDS (IAS) στο Παρίσι της Γαλλίας. Αυτά τα αποτελέσματα επιβεβαίωσαν ότι το STR χορηγούμενο μία φορά την ημέρα είναι βιοϊσοδύναμο με τη συνδυασμένη χορήγηση των επιμέρους παραγόντων, καθώς και ότι το STR είναι καλά ανεκτό.

Επιπλέον, τα αποτελέσματα από την βασική μελέτη Φάσης 3 EMERALD που παρουσιάστηκαν στην IAS έδειξαν ότι το χορηγούμενο μία φορά την ημέρα STR που περιέχει δαρουναβίρη 800 mg, κομπισιστάτη 150 mg, εμτρισιταβίνη 200 mg και τενοφοβίρη αλαφεναμίδη 10 mg [D/C/F/TAF] είχε χαμηλό αθροιστικό ποσοστό ιολογικής διαφυγής και υψηλό ποσοστό ιολογικής καταστολής στις 24 εβδομάδες σε HIV-1 θετικούς, ιολογικά κατεσταλμένους ενήλικες που άλλαξαν θεραπεία από ένα τυπικό σχήμα με ενισχυμένο αναστολέα πρωτεάσης (PI). Αυτή τη στιγμή βρίσκεται σε εξέλιξη ένα κλινικό πρόγραμμα Φάσης 3 για τη διερεύνηση της αποτελεσματικότητας και της ασφάλειας του STR με βάση τη δαρουναβίρη. Στις 20 Ιουλίου, η Ευρωπαϊκή Επιτροπή Φαρμακευτικών Προϊόντων για Χρήση στον Άνθρωπο (CHMP) του Ευρωπαϊκού Οργανισμού Φαρμάκων (EMA) εξέδωσε θετική γνωμοδότηση για το D/C/F/TAF5. Αυτή η επακόλουθη έγκριση από την Ευρωπαϊκή Επιτροπή επιτρέπει στη Janssen να κυκλοφορήσει εμπορικά το D/C/F/TAF σε όλες τις χώρες της Ευρωπαϊκής Ένωσης, καθώς και στις χώρες του Ευρωπαϊκού Οικονομικού Χώρου1.

Σχετικά με το D/C/F/TAF

Στην Ευρωπαϊκή Ένωση το D/C/F/TAF ενδείκνυται για την αντιμετώπιση της λοίμωξης από τον ιό της ανθρώπινης ανοσοανεπάρκειας τύπου 1 (HIV-1) σε ενήλικες και εφήβους (ηλικίας 12 ετών και άνω με σωματικό βάρος τουλάχιστον 40 kg). Ο γονοτυπικός έλεγχος θα πρέπει να καθοδηγεί τη χρήση του D/C/F/TAF.

Το D/C/F/TAF είναι ο συνδυασμός σταθερής δόσης τεσσάρων δραστικών ουσιών (δαρουναβίρης/κομπισιστάτης/εμτρισιταβίνης/τενοφοβίρης αλαφεναμίδης) διαθέσιμο ως επικαλυμμένο με λεπτό υμένιο δισκίο των 800 mg/150 mg/200 mg/10 mg. Η δαρουναβίρη αναστέλλει την HIV πρωτεάση και παρεμποδίζει το σχηματισμό ώριμων μολυσματικών σωματίων του ιού. Η εμτρισιταβίνη και τενοφοβίρη αλαφεναμίδη είναι υποστρώματα και ανταγωνιστικοί αναστολείς της ανάστροφης μεταγραφάσης του HIV. Μετά από φωσφορυλίωση, ενσωματώνονται στο ιικό DNA, οδηγώντας στον τερματισμό της αλύσου του DNA. Η κομπισιστάτη ενισχύει τη συστηματική έκθεση στη δαρουναβίρη και δεν έχει άμεση αντιική δραστικότητα.

European Commission Approves Symtuza® for the Treatment of HIV-1 In Adults and Adolescents in Europe

 

  • Symtuza® is the only darunavir-based single-tablet regimen (STR) approved for the treatment of HIV-1 in the European Union

Beerse, Belgium, 26 September 2017  Janssen-Cilag International NV (Janssen) today announced that the European Commission has approved the use of Symtuza® (darunavir/cobicistat/emtricitabine/tenofovir alafenamide [D/C/F/TAF])[1], a once-daily darunavir-based single-tablet regimen (STR), for the treatment of human immunodeficiency virus type 1 (HIV-1) infection in adults and adolescents aged 12 years and older with body weight of at least 40 kg. Cobicistat, emtricitabine and tenofovir alafenamide are from Gilead Sciences, Inc.

 

The only darunavir-based STR indicated for the treatment of this patient group, Symtuza® combines the proven efficacy and durability of darunavir with the improved renal laboratory and bone mineral density profile of F/TAF as compared to F/TDF (emtricitabine/tenofovir disoproxil fumarate). It is the only approved treatment to offer the convenience of an STR alongside the high genetic barrier to resistance provided by darunavir.

“There are almost one million people in the European Union currently living with HIV. The availability of a single-treatment regimen with a high barrier to resistance mutations eliminates the need for separate tablets, reducing the burden of pills on daily life for patients, and helping them to achieve improved treatment adherence and viral suppression,” said Jean-Michel Molina, Professor of Infectious Diseases at the University of Paris Diderot.

Results from a bioequivalence study that compared Symtuza® with the combined administration of the separate agents darunavir [D] 800 mg, cobicistat [C] 150 mg, and emtricitabine/tenofovir alafenamide [FTC/TAF] 200 mg/10 mg fixed-dose combination were presented at the International AIDS Society (IAS) conference in Paris, France in July.[2] These results confirmed that the once-daily STR is bioequivalent to the combined administration of the separate agents, as well as demonstrating that the STR is well tolerated.

In addition, results from the pivotal Phase 3 EMERALD study presented at IAS showed that the once-daily STR containing darunavir 800 mg, cobicistat 150 mg, emtricitabine 200 mg and tenofovir alafenamide 10 mg [D/C/F/TAF] had a low cumulative virologic rebound rate and a high virologic suppression rate at 24 weeks in HIV-1 positive, virologically suppressed adults who switched from a standard boosted protease inhibitor (PI) plus tenofovir/emtricitabine regimen. A Phase 3 clinical trial programme investigating the efficacy and safety of the darunavir-based combination is ongoing. In October, EMERALD 48-week data will be presented at ID Week 2017 in San Diego, California, USA, and 48-week data from the Phase 3 AMBER trial in antiretroviral therapy (ART) naïve patients will be presented at the European AIDS Clinical Society (EACS) Conference in Milan, Italy.[3],[4]

On 20 July, the European Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) issued a positive opinion for Symtuza®.[5] This subsequent European Commission approval allows Janssen to market Symtuza® in all countries in the European Union and European Economic Area.1
